  • A copy of your undergraduate transcript, official or unofficial. You can fax (817-459-3355) or mail it to headquarters.
  • Photograph of applicant only submitted via e-mail in jpeg or gif format to scholarship@orderofomega.org for Omega Trends newsletter.
